You are working the evening shift as a float LPN on a high-observation unit (HOBS) that is a stepdown unit for the intensive care unit (ICU). This HOBS also gets emergency department admissions who do not meet the criteria for ICU admission. It is a busy unit with a team of registered nurses (RNs), LPNs, and UAPs who work well together. Because of the teamwork that has developed among the nursing staff, more is delegated to the UAPs than you noticed last time you were floated to HOBS. You also noticed that there seemed to be less checking on the work of the UAPs. The situation is causing you some concern. Remember: You are accountable, in total, for any task you delegate to another person. You become uncomfortable when you see a UAP getting a patient, who is on strict bedrest, out of bed to ambulate to the bathroom. Fortunately, you were there and were able to prevent what could have been a serious incident. The UAP seemed upset with you for stopping the patient from being ambulated but went on with her work. You have five very sick patients with many parameters of care for you to manage. The behavior and attitude of the UAP do not bode well for the evening. Back to the original question: What are you going to do?
