Maintaining an accounting procedure for monitoring and tracking PHI disclosures is not only required by the HIPAA Privacy Rule, but it also stipulates a specific standard with respect to such recordkeeping.  Per the right to request an accounting of disclosures, an individual has the right to receive an accounting of certain disclosures made by a CE. Evaluate this patient’s request for an accounting of disclosures.  Using the Privacy Rule’s standard, determine if the disclosure should be included in the accounting report given to the patient. Defend your answer by indicating why the disclosure should or should not be included in the accounting report given to the patient. Scenario:  You are the Release of Information Technician at Sinclair Memorial Clinic (SMC). SMC utilizes Epic, which is an EHR.
