He states that he is unable to bear any weight on his right foot. He came into the office wearing open toed sandals. He denies any history of injury to his right foot. On exam the vital signs are normal, but the patient appears in acute distress, he has normal pulses in all extremities, but the right first metatarsophalangeal joint is hot, tender, erythematous and swollen.
