In this section, you want to identify one structural determinant and explain how this specific structural determinant contributes to the health disparity and group that you have selected for your topic. Examples of structural determinants of health are structural racism, culture, environmental conditions, value systems, living conditions, working conditions, stigma,& discrimination, economic conditions, etc.) At the end of this section, be sure to include an APA reference citation for your peer-reviewed scholarly journal article that you used for this section. Interventions (use this heading) Use 3-5 complete sentences in this section. This section is worth 21 points. In this section, you need to identify a social or behavior theory/model from the PH 3000 Social/Behavioral Theory video lecture/PowerPoint slides (they can be found under the Resources tab in our Moodle course). The social or behavior theory/model that you choose should be one that you can use to create an intervention to reduce your health disparity problem. After identifying your social or behavior theory/model, briefly discuss an intervention that can be used to reduce the health disparity problem that you selected for this assignment. Make sure that the intervention that you discuss addresses the structural determinant that you identified in the previous section. At the end of this section, be sure to include an APA reference citation for your peer- reviewed scholarly journal article that you used for this section. *Since you include the APA reference citations at the end of each section, you do not need to include an APA reference list.l
