Continuum of Care
For this Competency Assessment, you will examine the continuum of care a patient may need and research the services available in your area to support the patient. You will convey your findings in a compelling presentation to your colleagues.
To develop the content for your presentation, record detailed notes as you complete the following steps:
Review the information about the continuum of care and coordination among various health services organizations in the Resources, including the HIMSS resource “Continuum of Care” section, and the tables in Chapter 1 of the Shi and Singh textbook.
Read the HD1006 Patient Information document and select one (1), hypothetical patient, on which to focus for this Assessment. Assume your selected patient lives in your local area. If you are an international student, you may choose a geographic area in the United States or use your own location.
Use the pertinent information in Chapters 7, 8, 9, and/or 10 of the Shi and Singh textbook, and conduct additional research of your own to analyze specific services your hypothetical patient might need, beginning with the onset or diagnosis of an illness or injury and concluding with an end-point of your choosing (i.e., recovery, symptom management, end of life).
Research the types of organizations that provide those services.
Investigate the healthcare organizations in your local area. Based on the services your hypothetical patient needs, select four (4) actual settings in which he/she would receive care.
Next, gather information related to the types of insurance each organization accepts. Typically, this would include Medicare, Medicaid, and various types of insurance programs. What challenges related to payment for services do you anticipate your selected patient may experience?
Review the resources on the Institute of Medicine (IOM) domains of quality and consider what it means for healthcare to be safe, effective, patient-centered, timely, efficient, and equitable. Based on your hypothetical patient’s needs, analyze why professionals in healthcare settings need to be aware of the six Institute of Medicine Aims.

Describe a potential challenge related to payment for services this patient may encounter.
Describe a potential challenge related to the quality of care for services this patient may encounter.

Specific requirements to consider within your PowerPoint presentation include:
Text should be adequately sized and consistently used. No more than 3 distinct types of fonts should be used.*
Text should enhance understanding but should be brief, using concise wording.*
Use images or graphics sparingly to help convey the message. They should be, clear (recommend vector graphics vs. bitmap), and consistent with the overall presentation aesthetic.* Images should be free of copyright restrictions.

PowerPoint slides and notes (PDF export of your PowerPoint slides with notes):
You will save your PowerPoint presentation as a PDF with the notes visible. This document will include a picture of the slide along with your presentation script (written in the notes section) of each slide. Create your PowerPoint presentation according to the assessment instructions and then create a script for your presentation and write the script in the notes section on each slide. Your script should be word-for-word what you would say in your presentation. After you write your script in the notes section of each slide, you need to save your submission as a PowerPoint with Notes in PDF format. Follow these steps to save your PowerPoint as a PDF.
How to Save a PowerPoint as a PDF with Notes:
Save your PPT with notes as a PDF using the correct “Save” options already in your PowerPoint editor. Follow these steps:
Open the File menu at the top of the toolbar and Select “Print”.
Under Printer-Select “Print to PDF”.
Find the Layout menu and click “Notes” or “Notes Page” from the dropdown menu.
Hit “Print”- hitting Print will allow you to name your PowerPoint and save it (this will not send it to the printer to print).
