Describe one common type of medical error or safety issue frequently encountered in healthcare (e.g., medication errors, falls, hospital-acquired infections, wrong-site surgery, or communication failures during handoffs).
Using a realistic clinical example, explain how this safety issue could occur and its potential impact on the patient.
Propose at least two specific strategies a baccalaureate-prepared nurse could implement to help prevent this error and promote a culture of safety.
Your initial post must be 300–350 words minimum. Support your discussion with at least two scholarly sources (textbook counts as one; the second should be from QSEN, ANA, or a peer-reviewed journal). Use APA 7th edition for in-text citations and the reference list.Response Requirements:
Reply substantively to at least two classmates. 2. Quality Improvement Proposal Outline
Develop a brief quality improvement (QI) proposal outline for a common patient safety issue on a medical-surgical unit.Patient Safety Issue Options (choose one):Reduction of patient falls
Prevention of hospital-acquired pressure injuries
Improvement in medication administration safety (reducing wrong-time or wrong-dose errors)
Enhancement of nurse-to-provider handoff communication
