Identify one significant barrier to effective interprofessional communication or collaboration commonly seen in healthcare settings (e.g., hierarchical culture, poor handoff practices, role confusion, time constraints, or differing professional languages).
Describe a realistic clinical scenario in which this barrier could negatively impact patient care.
Propose at least two evidence-based strategies a nurse could use to overcome this barrier and improve collaboration.
Your initial post must be 300–350 words minimum. Support your discussion with at least two scholarly sources (textbook counts as one). Use APA 7th edition for in-text citations and the reference list.Response Requirements:

Instructions:Part 1: SBAR Practice
Review the SBAR (Situation–Background–Assessment–Recommendation) communication tool. Then, using the patient scenario provided below, write a complete SBAR report as if you are calling the healthcare provider or handing off to another nurse.Patient Scenario:
Mrs. Maria Lopez, a 78-year-old female, was admitted yesterday with a diagnosis of acute exacerbation of heart failure. She has a history of hypertension, diabetes, and chronic kidney disease.
