The purpose of this assignment is to introduce individual program/treatment plan (IPP) as the final step in program design. Students will develop skills needed to conceptualize and write a treatment plan for a mock client receiving recreational therapy services at the comprehensive program that was developed earlier in the semester.

Individual Program/Treatment Plan (30 points)

??CPP and SPP:
State what CPP team you were a part of and what SPP you developed individually.

??General Information and Client History
Provide the client name (pseudo name), DOB, and background information about the client/case (write this like it is a case study example), and what referrals were made to the RT program.

??Client Assessment
Choose an intake assessment tool (refer to Appendix C in textbook and other sources) that you would choose to gather information about the client. Describe the background, reliability, validity, and administrative directions. Provide the rational for this assessment tool being appropriate for this type of agency and client population.

  Summarize the strengths and needs of your mock client for each of the following domains: physical, social, cognitive, emotional and activity based.
