Hello class, so this week the only challenge I really experienced was basically having very limited time to see patients. This is so because my preceptor got in late both days. Overall, however it was a good clinical experience I got to see a good amount of health physicals for kids in different age groups. Most of the age groups were from 3 months -2 years old. There were also a few physicals for teenagers as well.
2.) Describe the assessment of a patient, detailing the signs and symptoms (S&S), assessment, plan of care, and at least 3 possible differential diagnosis with rationales.
Patients’ mom brought 3-year-old child R.M in for signs and symptoms of the cold like, stuffy/runny nose, sore throat, headache, mild fever as well as low appetite and not eating very well the past few days. Patient also says she has been having body mild body aches. Upon examination of the patient, she showed signs of a lot of congestion in the nose and a lot of mucous in the nose but it was not of green origin it was sticky and mildly yellow. Patient had no signs of labored breathing or use of stomach muscles upon breathing. There were also no signs of distress or lack of oxygenation as it was at 98% on room air. It was evident the patient had a fever as her forehead was warm and a tiny bit of pink on the cheeks. The plan of care for the patient would include to continue the Tylenol every 4 or 5 hours if needed or if fever rises above 100.5 degrees fare height. Advil should be given every 8 hours if fever is over 101 degrees fare height. For the patient’s body aches the Advil and Tylenol should help with this. Or if there’s still body aches the mom should massage the patient’s body to calm and or soothe them, heating pads can be contraindicated because of the fever. Patient should also drink a lot of fluids to prevent dehydration on the patient which can be very dangerous. So Pedialyte can be very beneficial as well in this case, as well as making sure to rest and get full nights rest. For the congestion nasal saline drops are beneficial. Making sure the patient also has hot foods like soup may help as well. Some of the possible differential diagnoses are the following: rhinovirus, influenza virus, and coronavirus (Lemieux J, 2022). Rhinovirus symptoms include coughing, sneezing, nasal congestion, and fever. Influenza virus is a acute respiratory infection symptoms are the same as for RSV. Coronavirus is another virus has the same symptoms as the RSV virus and it is as well anther respiratory virus.
3.) Mention the health promotion intervention for this patient.
The health promotion for this patient will include, making the child as comfortable as possible when home to allow them to rest and replenish their body to heal faster. Patient also needs to isolate and stay home a few days from school to avoid infecting other students as well as any family members in the home. It’s important to not use hot water or any steam humidifiers as this can cause the skin to scald.
4.) What did you learn from this week’s clinical experience that can beneficial for you as an advanced practice nurse?
From this week’s clinical experience, I learned a few key things to look for in patients of different age groups. I learned the measurements that we are looking for patients ages 3 months to 2 years old. As well as what percentile the patient is in and when to worry and consider referring them to a pediatric nutritionist or an endocrinologist. I like to learn this because I have a daughter myself and I might have more kids in the future so its beneficial to me as I can even do it in my own home.

RSV infection is typically a self-liming disease in otherwise healthy children. Thus, supportive treatment remains the mainstay of therapy (Moraes TJ, 2023). Supportive therapy means, hydration, cleansing of nasal secretions, as well as nutrition. In this case since the patient is overall healthy with no pre-existing conditions and no severe symptoms or signs of bacterial infection the overall plan of care involves supportive therapy as mentioned above.
