student named Alice. Karen indicated that this was a routine injection and that if
Jack had no objections, Alice would be giving the injection. Jack did not object and
rolled up his sleeve. Karen began charting the visit on the computer in the exam
room. Jack commented that there was some discomfort during the injection but
was assured by Karen that this was normal and Jack left the clinic.
Two weeks later, the pain in Jack’s shoulder had increased to the point that it
restricted his movement making it difficult to work and disturbing his sleep.

shoulder. He discussed with the doctor his feeling that this pain was related to the
pneumonia shot he received. He went on to file a claim against the clinic stating
that the student was not adequately supervised.
The Rest of the Story
On expert evaluation, it was found that the vaccine had been administered into the
joint instead of intramuscularly. The NP failed to supervise the medical student
sufficiently and assess the competence of the injection technique. The patient’s
medical records were insufficient stating only that the injection was given in the left
arm. There was no information on the batch number of the vaccine, and it was not
clear who had administered the injection. The patient required steroid treatment
and decompression surgery to treat the injury. This is an example of inappropriate
delegation, and the NP remains accountable and responsible for the injury that
resulted.
