This is hypothetical, so you will not actually be distributing these materials to a population group.The component of this project will include a document detailing the population and health issue assessment, theory selection and rationale, objectives for the theory-based educational materials, and a theory chart. Specifically, you should address the following things:

Health Issue Assessment: Explain what the health issue is and why this is an issue for your specific population.

Social Assessment: Identify the quality-of-life and social factors surrounding the health problem.

Epidemiological Assessment: Identify the prevalence/incidence of the health condition in the affected population.

Behavioral and Environmental Assessment: Identify the behavioral and environmental factors that are linked to the health issue.

Educational and Ecological Assessment: Identify the predisposing, enabling, and reinforcing factors linked to this health issue.

Theory Selection and Rationale: Identify the connections between the health issue/problem and the theory you will be using.

Goals and Objectives for the Educational Materials: List the goals and SMART objectives for your educational materials. For more information about SMART objectives, review this document.

Theory Constructs Chart: Each theoretical construct has a thorough explanation of how it is addressed in the materials (provides the information that is being used to address the construct). Here is an example of a Theory Constructs Chart.
