Stop the transfusion, return the unused blood to the laboratory and collect urine and blood samples urgently to determine if this is a haemolytic transfusion reaction Activate the medical emergency team as this is almost certainly a haemolytic transfusion reaction and he is likely to become critically unwell very soon Recognise the possibility of a transfusion reaction, stop the transfusion, maintain IV access, assess/manage symptoms, repeat clerical check of the pack and notify the clinical team/transfusion laboratory as needed Reassure the patient that without specific symptoms, he is unlikely to be experiencing a transfusion reaction. Tell him you will return in 30 minutes to complete his hourly vital signs but to call again if further symptoms develop
