During clinical this week, my assigned patient was an 85-year-old male that presented with aortic valve stenosis and etiology of cardiac valve disease unspecified. He was admitted to Mercy Fairfield on 06/06 and a transcatheter aortic valve replacement (TAVR) was performed at two groin sites to replace a narrowed aortic valve that fails to open properly.The patient has apast medical history of atrial fibrillation, hyperlipidemia, hypertension, hypothyroid,labyrinthitis, myelodysplastic syndrome (MDS), monoclonal gammopathy of undetermined significance, non-insulin dependent diabetes mellitus (NIDDM), renal cell carcinoma, and stage chronic renal impairment associated with type 2 diabetes mellitus. The patient is full code status, on a regular adult diet, has a condom catheter and has a peripheral IV on the left forearm that is clean, dry and intact. His allergies include nitrofurantoin and macrobid. His 8:00 am vitals are as follows: blood pressure 126/63, heart rate 82, respirations 16, 02 saturation level at 95% on room air, temporal temperature 97.3, and patient denied any pain. Patient’s blood sugar level was 197. The patient correlates to the clinical focus because patients with better care experiences often have better health outcomes. It is essential to understand the patient’s condition and diagnosis, procedures that are being performed and what interventions are being implemented.Without understanding these things, medical errors are likely to occur.
