If changes to scope-of practice regulations could help abate health worker shortages, why are changes not made? Which scope-of-practice proposals would you support?
Scope of practice defines the roles of healthcare professionals and what activities they can legally perform by law based on their education and training. For example, there are certain tasks that can be performed only by RNs and not by LPNs. Due to healthcare worker shortages, many health policy experts have advocated for increased roles of non-physicians and other allied healthcare workers (Knickman et Elbel, 2019).
One reason changes may not be made to scope-of-practice regulations is due to performance-based payment programs. These programs give facilities financial incentives to focus on models of care that increase the quality of care while maintaining a reasonable cost. For instance, higher RN staffing levels are associated with better patient outcomes (Knickman et Elbel,2019).
I would support a proposal to change to scope of practice for LPNs. I have worked closely with LPNS and learned much from their knowledge and experience. Based on the LPNs experience and length of time in the field they should be able to perform some of the same duties of an RN. I would recommend check off demonstrations of certain skills to qualify them to perform these skills under RN supervision. If an LPN can correctly demonstrate a skill under RN observation, they should be able to perform that skill as well. This could be a cost-efficient solution to staffing issues.
