You observe it being performed regularly in the setting where you are currently assigned, a place with a good reputation (apparently well-deserved, generally speaking). In fact, as you observe patients’ responses to the procedure, you become convinced yourself of good reasons not to use it. Now you have only 1 week left in this rotation and raise your concern with your supervisor. She responds, “Actually, we know that, and we don’t like it either. Our health plan, however, does not allow us to use the newer procedure because it is four times as expensive as this one.”

Step 4: Explore the practical alternatives. List and describe all alternatives that you can think of for this situation. Do not exclude possible solutions that you may not support. In other words, I want to see everything you can think of even if it is impractical. When you have completed the list of alternatives, tell which you would choose and explain why.
