Your client, Thomas, is up for review. He has made some significant progress with regard to quitting smoking and exercising. However, he continues to struggle with mental health. He refuses to go to counseling because well “men can do this on their own”.
Meet Thomas
Thomas is a 52-year-old African American male. Thomas is a retired Army Sergeant and currently works part time as a contractor with the United States Military. He teaches new recruits. Thomas smokes 1-2 packs of cigarettes a day. Since Thomas retired from the military, he has put on about 30 pounds. Thomas has stopped exercising since he left the military. Thomas is married and has 2 grown children. He reports that his relationship with his wife is tenuous at best.
Thomas has developed a drinking problem since he left the military. He indulges in binge drinking. This binge drinking does affect his work, but it is a part time job and Thomas is often heard asserting, “I don’t need this job anyway.” Thomas has mood swings and his mood swings have grown more severe, and he alienated the people around him. His problem was accentuated by his abuse of alcohol and other drugs. His boss has told him that he needs to settle down or he will lose his job. Thomas’s wife indicates that his moods have become difficult to deal with. He is happy and exhilarating one minute and crashes into devastating depression, during which he would withdraw from all social contact, refuse to eat, and lie in bed crying for days at a time.
While in the military, Thomas experienced four deployments to active war zones, but insists that the deployments did not affect him. “It is part of the job,” is his token response to his any discussion of war time activities. Thomas has never sought counseling, nor has he considered it. Recently, his children and his wife have told him that he needs help. However, Thomas refuses help. In the last week, Thomas was admitted to the hospital with complaints of chest pains and panic. Thomas has been diagnosed with Post Traumatic Stress Disorder.
