Preoperative Diagnosis: Left carotid aneurysm. internal extracranial portion Postoperative Diagnosis: Same Procedure: Left carotid artery test occlusion Procedure Description: With the patient properly prepared and draped in sterile fashion. a 6-French sheath was inserted in antegrade fashion into the right femoral artery. A diagnostic catheter was inserted coaxially through the sheath and used to select the left internal carotid artery. An exchange 018 wire was placed through the catheter and then the catheter was removed. A 6 mm x 2 cm balloon catheter was threaded over the wire and positioned with the balloon across the lumen of the proximal left internal carotid artery. The balloon was inflated to 0.5 atmosphere while slowly injecting contrast through the end of the balloon around the wire. Complete stasis of the carotid artery was documented. Neurologic testing was performed for five minutes. The patient was placed on heparin prior to this procedure, and a 5.000-unlt bolus was administered in the left carotid artery just prior to balloon inflation. Another 1,000 units of heparin were slowly administered through the tip of the balloon over a five-minute period, along with saline. Only a small amount of total fluid was administered. and contrast was found to be static in the carotid artery during the entire period. After five minutes. the balloon was deflated and the patient was sent to the recovery area in good condition.
