I met with my client July 14, 2022 at 4 pm in the community. I felt so bad cause she told me she’s been feeling very depressed. She said she thinks she needs to be admitted to the psychiatric unit. I panic a bit cause I get nervous over this kind of thing. She then tells me that she has had negative experiences going through the emergency department in the past. Understandably, she wants to know if she can skip the whole emergency process and go straight to her unit. So I have to break the news to her that this won’t be possible. I explain that this would have to be something that is prearranged with the psychiatrist however, even if it is prearranged, it is still dependent on bed availability. I really tried to emphasize with her frustration. I know I’d be frustrated! I also commended her self-awareness about her needs. Not all clients are that in touch with what they need!
Then she started to cry about how it is unfair. So I figure it would be helpful to share that I have worked with many people who do not like going to emergency. I comment on how it is unfortunate that people don’t feel it is a safe place. I talk about the fact that she brought up her needs and ask how she can have her needs met with the obstacle she has identified. I was pleased with the wording of this question! My client says she can ask her sister to go with her. She says that she doesn’t normally talk to her sister about these things but if she opened up to her, maybe it would help to have somebody with her. She stops to talk for a while about her family dynamics and how her mother and uncle sometimes go years without talking. She comments how she does not want this to happen to her and her sister. I remember these details cause I can relate to the family dynamics.
I ask whether she may like you to write letter of support. I tell her I could send it to the charge nurse prior to attending emergency with her sister so they understand how difficult this is for her. Hey, it’s worth a try! My client said yes. We do up the consent. I then write the letter with my client’s assistance and let her know I will fax it after our meeting. I also do safety assessment and she confirms she is not at risk of ending her life. Such a relief. Hope she gets the help she needs.
My client says that she will be asking her sister to attend the hospital with her when she gets home. She then says she feels hopeful as a result of confiding in me. So nice to know! So I set up a phone meeting for the next day at 2:30 pm to find out whether she has been admitted.
