The process steps are pre-listed in column one. In column two, list what could go wrong (the potential failures) at each of the process steps. There is space on the form to list two failures (one per row) for each process step. For each failure listed in column two, determine what would happen if the failure occurred (the effect) and record this in column three. Each failure should have an effect recorded. Using the risk assessment matrix provided with the answer sheet, rate the likelihood of each failure you’ve listed in column two. The likelihood rating is a numeric score, ranging from 1 to 6. Put the likelihood score for each failure in column four. Using the risk assessment scoring matrix provided with the answer sheet, rate the consequence of each effect you’ve listed in column three. The consequence rating is an alpha score, ranging from A to D. Put the consequence score for each effect in column five. Using the risk assessment matrix provided with the answer sheet, rate each failure listed in column two as High, Medium, or Low. This rating is obtained by determining where the frequency and consequence scores for the failure intersect on the matrix. Record the rating for each failure in the last column. Select two of the High or Medium failures and write a short description of what could be done to prevent each failure from occurring.
