Three components of the Public Health Nursing (PHN) standards include assessment, population diagnoses and health education and promotion (American Nurses Association, 2007). My practicum setting is a non-profit community hospital that serves patients with numerous aliments. My practicum experience will focus on the diabetic population. At my facility, patients with new onset and uncontrolled diabetes are provided with treatment, education on management of disease, and barriers to care are explored (e.g., lack of health insurance, language barriers) and a plan is developed, including community referrals are set up. Education is highly important in my practicum setting. Individuals are educated on lifestyle changes such as exercise, diet, and blood glucose monitoring that are necessary to maintain good health status related to the management of their disease. To formulate a plan, is essential to first complete and assessment and diagnosis of the individual and community to determine specific needs and barriers to care.
The American Nurses Association (2013) emphasize that public health nurses must be strong at assessing potential or actual assets, needs, opportunities, and inequities of individuals, families, and populations and translate this assessment into action for public good. Additionally, my practicum site will allow me to collaborate with a diverse group of healthcare professionals, such as diabetic educators, dieticians, as well as learning how to work with community organizations and identifying state healthcare benefits and resources that are available to assist individuals in their healthcare journey.
