The care plan was therefore presented to the patient straightforwardly and understandably. The patient was motivated to engage in the plan since he was given a clear image of the treatment phases.
At the implementation stage, a care plan was followed by nurses, patients, their caregivers, and other multidisciplinary team members as necessary (Toney-Butler & Thayer., 2022). They made sure that the necessary actions were put in place so that the patient may accomplish their objectives. The most crucial step of the procedure was the assessment phase, which informed the patient, their carer(s), and the multidisciplinary team of whether objectives have been met or are being worked towards. Future activities were decided upon via evaluation. Setting attainable objectives helped the patient feel motivated rather than disheartened. For instance, they create brief, positive objectives instead of reporting regression.
According to surveys completed as a hospital study, many patients had a favorable opinion of integrated care. Patients often felt neglected, and their symptoms were discounted when they lacked a comparable objective assessment.
