You are a professional nurse and the assigned team leader on a busy medical surgical unit at a busy teaching hospital. You routinely care for patients and perform at least 1 admission and 1 discharge each shift. Your nursing team typically consists of yourself, a licensed vocational nurse or LVN, and 1 unlicensed assistive personnel or UAP. The LVN is assigned to 4 of the patients and the UAP is assisting patients with activities of daily living.
Each morning at 7 30 a m medical residents and a pharmacist make rounds and write orders. While the medical residents and pharmacist make rounds, you are busy assessing blood glucose levels on 3 patients and administering insulin before the breakfast trays arrive. The medical team often has orders they want carried out and often want you to accompany them on rounds to answers their specific questions, as well as to complete and write orders about the patients.
This morning, the rounding medical residents want you draw blood cultures on one of the patients, to contact the same patients’ assigned social worker and case manager as well as to contact the floors’ patient care coordinator to begin discharge planning on another patient. You inform the medical resident and pharmacist you are busy and will complete the order as soon as you finish blood glucose levels and administering the insulin.
At the same time, physical therapy arrives and wants to ambulate 1 of your assigned patients but asks you to administer a pain medication first before beginning to ambulate the patient. You tell the physical therapist to reschedule the ambulation and that you will ensure the pain medication is given before the appointment.
Later in the morning, your clinical nurse manager asks to speak with you regarding a report from the medical residents, pharmacist and physical therapist that you were abrupt and uncooperative. You explain what happened this morning and that this type of situation happens most mornings when the medical residents and pharmacists are rounding and when the physical therapists decides to ambulate one of their patients last minute.
You go on to explain to the clinical nurse manager, you are concerned the inter profession team members do not understand your role as a team leader and your role as a registered nurse on the inter professional team as well as what takes place on a busy medical surgical unit. Your clinical nurse manager calls a meeting and wants you to prepare for the meeting and to lead it.
