Using a variety of instruments (Windshield Survey, Community Assessment Tool, Screening interviews, etc.) while at your clinical site, you will assess the community location and people whom the agency serves. This will lead to a list of identified, prioritized health needs and your recommendations for intervention.

Familiarize yourself with the instruments for a Windshield Survey (Website Resource 15C, p. 425) and The Community Assessment Tool applied to Phenomenological Communities (Website Resource 15A, p. 425). You may also wish to review Chapter 13, Box 13-1 (p. 343) for examples of assessments that may be appropriate for your community agencys population, since the Community Assessment Tool includes some screening data and information from clients.
Plan how you will obtain the information for the assessments.
Conduct a Windshield Survey assessment during Clinical Week 3.
Conduct interviews with clients and/or families from the agency during Clinical Week 4.
Complete the phenomenological community assessment during Clinical Week 5.
Document the assessment results, using the format and prompts from the Community Assessment Tool. Incorporate the Windshield Survey information where most appropriate in the Community Assessment.
Create a prioritized list of health needs for the community.
Identify primary, secondary, and tertiary prevention strategies for the top 3 community health needs, citing any references in APA format.
Make recommendations for individual, family, group, and/or community interventions for the top 3 community health needs, citing any references
Provide a reference list in APA format.
Submit your completed project on Week 5

Develop an evaluation plan for your teaching intervention and create a tool for feedback from your learners.
Implement the teaching plan and evaluation during Visit 5 of your clinical experience.
Summarize the outcomes of your teaching plan and evaluation.
Submit your teaching plan, evaluation tool, and summary on Week 7. Zoom meeting for presentation will be on week 7.
