Community Resources for Aging Population

For this assignment, you will need to identify three resources within your local community that cater to the aging, disabled or chronically ill. The first resource needs to relate to a physical need (e.g., nutrition), the second resource needs to relate to a psychosocial need, and the third resource needs to be related to transportation or entertainment needs. You may not use the same organization or entity for more than one area (i.e., you cannot say ABC company provides meals and transportation so it meets #1 and #3. You would only be able to use it under one category – either 1 or 3).

All sections must be complete, accurate, and using scholarly writing (e.g., complete sentences, proper grammar, etc.). You must include complete references to show where you located the data. The boxes will expand as you type.

As this is information that you may use in your work setting, it is best to examine your local community. However, if services are minimal, you may wish to expand your search to the county or nearest city, depending upon your location.
