Discussion prompt reply: Interprofessional collaboration helps prevent medication errors and morbidity and mortality rates. Collaboration is extremely beneficial for the patients. The most common interprofessional collaboration I can think of are registered nurse, physicians, NPs, RTs, OT, PT, social work, and case management. In the hospital setting these unique individuals all work together for the good of the patients. For the interprofessional collaboration to be affected there should be role clarity. Each member should understand his/her unique role (Bosch & Mansell, 2015). There should also be respect among each member of the interdisciplinary team. Teamwork, trust, and confidence are also important because there will times when challenges surface and it is up to the team to work together to help the patient.
In order to be an APRN it is important to have an intense set of clinical skills, knowledge and integrative abilities. One may think that since it is expected of an APRN to be intelligent and to embody a large about of knowledge the APRN must know everything. That is far from the truth. It is impossible to have all the answers, and that is why it is important to refer a patient to another provider if the help cannot be provided at your facility. For example, a female patient comes to the clinic with 9/10 abdominal pain and has a menstrual cycle that has been extremely heavy this month. The APRN can ask questions, and possible do a blood test to check H&H, perform a pap smear and do an abdominal ultrasound. What if the pap smear came back abnormal? It is important for the patient to be referred to a gynecologist or OBGYN because that individual specializes in that area of the body. Referrals to different providers is apart of interprofessional collaboration. Another example of interprofessional collaboration is if a patient has chest pain and you may think he/she had a heart attack. Some clinics do not have access to an EKG machine and possibly are not able to test for troponin levels. It is important to send that patient to the ED and also refer the patient to a cardiologist. Another example is an elderly patient who is declining and may need to go on hospice. Setting up hospice for the patient is also interprofessional collaboration.
With interprofessional collaboration it is important to make sure the patient has an updated problem list and medication list. The other facilities need to know the past medical history of the patient and all the medications the patient is on, so they won’t prescribe meds that are contraindicated or prescribe meds that the patient is already taking.
