During Consultant Chat #2, you will collect information about your Consultant and use that information to create a Medical Emergency ID Wallet Card. Collect the following information in a personal face to face home visit for your journal entry and the Medical ID card:
Full Name
Address
Email (YES, some older people really DO communicate via email!)
Phone Number
Date of Birth
Physician Information (name, phone)
Emergency Contacts (first and last name, phone, relationships)
Medications / Supplements ( name, dosage & frequency, reason for taking it-existing medical condition)
Allergies and reactions
Other Information (advanced directive, blood type, Insurance Provider, preferred hospital)
After collecting the above information:
1. Use a piece of card stock paper to print out the Medical Emergency ID WalletPreview the document card.
2. Enter your consultant’s information on form provided.
3. Be sure to leave the completed card with your consultant.
Consultant Chat #2: Scripted Questions (Also for your journal entry!):
I will be assisting you with filling out a wallet card that you can keep. Your information will be kept confidential between you and me. Use the Medical Emergency ID Wallet Card information above for this question.
Next, ask the following questions and put them in your journal entry after completing the medical card:
1. Did you receive a completed Medical Card from your student? Do you think it will be useful to you?
2. How do you pay for medicine? Is the expense for your medications a burden on your budget?
3. Do you go to the pharmacy to get your medications or have mail order delivery? Did you know there is a law that says that a licensed pharmacist MUST do an education session with every patient who comes to a pharmacy to get a prescription filled?
4. What health insurance coverage do you have?
5. How have the changes in Medicare affected you?
Consultant Chat 2 Journal Entry
1. post the answers to the scripted questions 1-5 in your journal entry.
2. Remember to include your critical reflections at the end of the journal entry for full credit.
Post what you have learned from your consultant in the journal entry of at least 250 words (not including the questions) for credit. See rubric.
